What did the ending mean in Once Upon a Time in America?
The sequence then suggests that everything that happened after Noodles entered the den –– his escape to Buffalo, his return to New York, Max's rise to prominence and impending fall –– was a product of an opium-induced dream.
At the end of Sergio Leone's Once Upon A Time In America (1984), Max (James Woods) appears finally to be dead. Max, alias Secretary Bailey, Secretary of Commerce, has been killed on the eve of testifying before a Congressional Investigating Committee, late in 1968.
Elements of the 1968 story, particularly its open ending, are a bit too surreal to be wholly acceptable as to what “really” happened. While never confirmed, there is a strong implication by the end that the film has been an opium dream, Noodles' memories of his past mingled with fantasies of a possible future.
If Once Upon a Time in Hollywood is about Sharon Tate in any sense, it's about giving her that life back, and studiously not fixating on her death. To some extent, it's actually about leaving her alone. Tate's sister Debra approved and endorsed both Robbie and the film as a whole.
Noodles meets with Max in his private room at the party. Max explains that corrupt policemen helped him fake his own death, so that he could steal the gang's money and steal Noodles' love interest Deborah, in order to begin a new life as Mr. Bailey, a man with contacts to the teamsters' union.
Maybe it's inside that smile that Noodles imagines what we see in the movie, like an unconscious projections of his wish that his friend is still alive, and he should't have any remorse about his death.
Plot holes
As the film moves through the years to 1933 then to 1968, several of the gang have been killed. After Max, Cockeye and Patsy are supposedly killed in the 1933 ambush, Noodles goes to the safety box to look for the money -- its not there. He spends 35 years in Buffalo trying to figure out what happened to it.
Later on, it's implied that Max conspired with Monaldi to kill Noodles after Noodles refused to get involved with corrupting the Teamsters Union and refusing to take part in a New York Federal Reserve heist.
Charming tells Henry that they will find a way to get Emma and Snow/Mary Margaret back, together. Later that day, the Mad Hatter is finally reunited with his daughter, Paige/Grace, and Henry watches this from a distance.
He then aimed to derail Henry's pursuit of Cinderella and take claim the other Hook's life and wife. Did Max survive the coma?
The fan-favorite character ended the season in a coma with her bones broken and eyes blinded at the hands of Vecna. Max technically died for one whole minute before Eleven revived her friend, but Eleven's powers weren't enough to bring Max fully back to life.
Once Upon a Time in America is the final entry in Leone's “Once Upon a Time” trilogy. It followed Once Upon a Time in the West (C'era Una Volta Il West) (1968), and Once Upon a Time in the Revolution, which came out in 1971 as Duck, You Sucker!.

Cuts were made to the two rape scenes, and some of the violence at the beginning. The final flashback montage of Max and Noodles as children was also eliminated from their final scene.
